Forms
There are three prescribed forms and seven approved forms.

Prescribed forms
The text of the three prescribed forms must be exactly as contained in the three corresponding Schedules of the Owners Corporations Regulations 2007, without additions or deletions.

• Schedule 1 - Proxy Form - use this form when appointing a proxy - Word format | PDF format
• Schedule 2 - Model Rules for An Owners Corporation - these rules apply by default to an owners corporation unless the owners corporation makes its own rules and registers them with Land Victoria - Word format | PDF format
• Schedule 3 - Statement of Advice and Information for Prospective Purchasers and Lot Owners - this statement must be attached to the vendors statement or section 32 that is prepared when a property is being sold - Word format | PDF format

Approved forms
The seven approved forms must be used for particular processes undertaken by or in an owners corporation. The layout of the forms can be varied to suit computer generated systems, mailing and payment processes but must contain all information specified in the approved forms.

• Fee Notice - Sent from an owners corporation to lot owners - Word format | PDF format
• Final Fee Notice - Sent from an owners corporation to lot owners - Word format | PDF format
• Contract of Appointment of a Manager - Used by the owners corporation when appointing a manager - Word format | PDF format
• Complaint Form - Used by a lot owner, tenant or manager to notify the owners corporation of a breach of the Rules, Owners Corporations Regulations 2007 or Owners Corporations Act 2006 - Word format | PDF format
• Notice to Rectify Breach - Used by an owners corporation to request a lot owner, tenant or manager to rectify an alleged breach of the Rules, Regulations or Act - Word format | PDF format
• Final Notice to Rectify Breach - Used by the owners corporation as the final notice to rectify a breach - Word format | PDF format
• Notice of a Change to the Rules - Used by the owners corporation to notify Land Victoria of the making, amendment or revocation of an owners corporations rules. NB - this is a Land Victoria form - Word format | PDF format

Fact sheets
There are 20 fact sheets that provide information and assistance in living in and managing an owners corporation. Links are also provided to forms and pro forma documents that may be useful. Approved and prescribed forms must be used for particular processes undertaken by or in an owners corporation and their text cannot be changed. Pro forma documents are provided for information and assistance, do not have to be used by your owners corporation, and you are free to amend them to make them more useful for your owners corporation.
