Annual statement reporting - patriotic funds

We will mail you a 'Patriotic fund annual statement' form to complete each December. 

Note: these forms are not available before this time. The statements are barcoded, specifying the fund and year. 

You must return the form by 31 March the following year (or by another date approved by the director) and include a fund:

  • income and expenditure statement for the year ending on the specified date (usually 31 December of the previous year)
  • balance sheet as at the specified date.

If you lose your 'Patriotic fund annual statement' form and want another copy, email
