The information on this page is for SDA providers. It explains when and how you must notify us (CAV) and other agencies about SDA residency agreements and notices.
SDA residency agreement
You must notify us within 14 days of entering into or establishing an SDA residency agreement.

What you will need to include:
- Provider registration number (PRN)
- Provider name
- Provider address
- Email address
- A mobile or business phone number
- Property address
- Agreement start date
- Agreement end date
SDA notices
Some notices must be reported to us, whether you give them to or receive them from your resident. In some instances, you will also need to report them to the National Disability Insurance Agency (NDIA).

You must provide us with a copy of the following notices within 24 hours:
If your SDA resident is an SDA recipient (that is, they are funded by the NDIS to reside in an SDA enrolled dwelling), you must also provide a copy of the notices above to the NDIA within 24 hours.
After you send a copy to us, we will send a copy to the Office of the Public Advocate. If applicable, we will also provide a copy to the Transport Accident Commission or WorkSafe Victoria.
