SDA residency agreement
If you enter into or establish an SDA residency agreement, you must notify us within 14 days.
Notify us by completing this form:
Add a new SDA residency agreement
Submit one form for each SDA residency agreement you enter into or establish. Make sure you have these details:
- Provider registration number (PRN)
- Provider name
- Provider address
- Email address
- A mobile or business phone number
- Property address
- Agreement start date (must be after 1 July 2019)
- Agreement end date (must be after 1 July 2019)
SDA notices
You must notify Director of Consumer Affairs Victoria (CAV) if you give your resident certain notices and if your resident gives you certain notices.
Notify the Director of CAV by completing this form:

Notify the Director of CAV within 24 hours
You must notify the Director of CAV within 24 hours if you give your resident a:
- Notice of temporary relocation
- Notice to vacate
You must also notify the National Disability Insurance Agency (NDIA) if you give any of the notices listed above to an SDA resident who is an NDIS participant, funded to reside in an SDA enrolled dwelling.
After receiving a notification, the Director of CAV will provide the details of the notice to other relevant funding agencies, including the Office of the Public Advocate, the Transport Accident Commission and the Victorian WorkCover Authority.
You must notify the Director of CAV within 24 hours if your resident gives you a:
- Notice of intention to vacate
- Notice of intention to terminate
You must also notify the Office of the Public Advocate if you receive any notices listed above from an SDA resident. If the resident is an SDA recipient, you must also notify the NDIA.
Notify the Director of CAV within 14 days
You must notify the Director of CAV within 14 days if you give your resident, or your resident gives you, a Notice of withdrawal.
